I have received a PIP review form but I can't tell if it's the old one or new one so I don't know what guide to use. I can't see a date at the bottom left hand corner, it's just 2 sets of numbers. I wasnt sure whether to add the numbers in my query incase im giving away some kind of personal information. Could someone please advise (and I will provide the numbers if it helps you answer my question). thank you.
Also I have only been given 2.5 weeks to get the form back to them but it will take me alot longer than that due to health issues and finding someone to support me. Is there a way to get them to extend the time without having to phone (as I have hearing issues and mental health problems and usually correspond with people via email/writing/messenger etc), an email address perhaps? I dont think writing will be a good idea because it took them 6 weeks to get back to me the last time.
I would look at the guides then look at the review form.
I have not seen any of the old review forms for a while so I would hedge my bets on the new guide.
If you need an extension in time then contact the DWP before the review form has to be back and ask for an extension. You will need to phone them or get a friend/family member to phone to get an extension, if you do it first thing in the morning then you usually don't have to wait for long on the phone.
Don't forget to send the review form back by recorded delivery.
