I had my telephone assessment last Wednesday and called on Friday for a copy of the report which arrived today, the main reason for the assessment was that I was diagnosed with Mixed anxiety and depressive disorder on April so I had informed them of a change as was already getting the standard rate on daily and mobility. After reading her report I disagree agree with a few things she has said like I can plan and follow a route unaided even though I said I suffer extreme panic attacks and can no longer drive or leavethe house (I did record telephone conversation) should I query the report or await till I receive the official decision

You have a small window of time to write to the Decision Maker before the decision letter is sent in which you can challenge the report. Don't say that you recorded the conversation, as it will be ignored. Sometimes this works, and sometimes it doesn't, but you have nothing to lose at this point, and you will still have the opportunity to put in for a mandatory reconsideration if you want to challenge the final decision. Address your letter to the 'Decision Maker'.
