I received my PIP review form today and I am confused about which guide to use to help me fill it in. There is no date in the bottom left of the form, just a series of numbers which I don't understand. I have looked at both guides and none of the questions "line up" with the ones in my form, but they are similar.
Can anyone advise me as to which one to use?

P.S. For completeness, the numbers on the left hand corner of the front page are:
001207/000010/000028 01298-00028-001-001-01
After the front page, only the second line of numbers appears on each page.

You need to use the first PIP guide for the new guide which can out in January last year, the second PIP guide is for the older style form.
Remember to treat the review form as if it was a new claims form, we do not recommend typing no change, if you kept a copy of your original claim then use that as a template, if there are any change to your condition then add these changes and explain how these changes affect you and enclose supporting documents old and new if they support your claim.
