Hiya, still awaiting my decision but having got a copy of my PA3 am horrified at the lies and inaccuracies. Mine was changed from a video to a phone call on my mobile by assessor at time of call. There is a whole section in there of observations saying assessment undertaken by video link. Says normal manner and facial expressions, did not look tired, not anxious , restless, or tense. Says average build, well kept, casually dressed looked well ……. Should I go on , 12 lines of observations .
I am appalled at this as it’s simply untrue and the assessor never saw me should I complain before I even receive my decision am so upset and angry at this and at a total loss.

I feel your anger! I would definitely write back and complain. I would phone as well first.
You could write in and say
[Heading]
Complaint: False PIP Assessment Report
I wish to complain about the report, which doesn't appear to be my report. I had a phone assessment but yet the report refers to a video assessment and observations made via the video link. The report therefore can not be correct.
[add as appropriate, and the comments do not refer to my situation ]
There would appear to be three alternative possibilities:
•There has been a mix up and the report belongs to someone else,
OR
•2 reports have become muddled together
OR
•the report has been deliberately fabricated
I would therefore be grateful if you could confirm in writing that given the circumstances you will not be relying on the report.
I would also be grateful for a thorough investigation to be made as to what has happened with the reporting of my assessment.
Please provide a full explanation of what investigations have been made.
In the event that the Assessor has made a false statement I would like a full explanation as to what disciplinary steps are being taken against this Assessor to ensure this error does not occur again.
Keep it snappy! No need to go into any great detail. Send to both DWP and the complaints department. Include NI Number and name on all pages.
PS I suspect it very unlikely that DWP will admit the Assessor had falsified the report. (And I guess it is possible there has been a computer glitch!) If DWP relying on the report despite your letter, then escalate the complaint. Be aware given the clear inaccuracies in the report you might be asked to undertake another assessment, but hopefully not. You need to send on the letter now.

You could also check the time lag between the day and time of your assessment and when the report was completed. There was a delay of several days in my case and I used this fact in the MR.
