Hi Owlofthemoon.
If I was in your position I would seriously consider making a complaint about your previous GP. Poor, incomplete or wrong record keeping can get a GP suspended and is considered a very serious offense by their governing body. If you do make a complaint you should make it to your Integrated Care Board. It is probably the reason why your appointment was abruptly ended.

Regarding your question about requesting a paper-based assessment and whether you are disadvantaged by having one. Obviously, if there is no chance for you to be questioned, the HP has to make a decision based on the information you send in. Sometimes that works very well for people, and for others, it doesn't. Having a telephone assessment gives you the chance to have your say - but in truth, this depends on the assessor and how well they do their job. There are plenty of people who think their assessor was really nice, and then when they see the report, it's full of errors. misconceptions and sometimes downright lies. Some people are fortunate and have a good experience. It shouldn't be, but so often, it is the luck of the draw. I can't tell you what you should do - I only offered up the idea of a paper-based assessment as a possibility if you can't cope with a telephone assessment. (The DWP might not even agree - even if you ask - but if they don't you do have the chance to appeal now, which you didn't in the past).

Thank you everyone - BIS, Gary, Mollie and Sue for all your inputs.
I submitted my application with detailed responses, a carer's letter and limited medical evidence but without a dairy as I am knackered and unable to make it meaningful for someone else reading it. I received a text to say they had received it last week.
I did request a paper assessment detailing overwhelming distress, physical pain and speech difficulties to the extent that I have been considering learning BSL. But I got a text from Independent Assessment Services saying they are looking into the case and may contact me for an assessment.
I am petrified and unsure of what's going on. Does this mean my request for a paper assessment has been rejected but I have not been informed and that I will have to undergo some other form of assessment ?
Has there been a change to the kind of assessments reported in the guide as 77% telephone.
Any ideas on this and potential timeline? Please could you advise ?

I would wait until you hear from them. If they say it is an assessment - then appeal and ask them to look at that decision. You mentioned that you don't have any input about your mental health - so they may push back and refuse, but you can only try.

Throughout the form I have referenced mental difficulties especially in engaging with people in the "communicating" question and the "planning a journey" question.
For the assessment question, I did not detail my physical, cognitive and mental difficulties just briefly mentioned as I had already written 'War and Peace' around them in my responses to the questions in the form.
Do you think this may put me at a disadvantage? Might it be worth sending an 'addendum' with more explanation?

I can't tell you what to do here because I have no idea what you wrote and how it linked to the specific questions. Most people, when they complete the forms, worry that they have missed something. Sometimes it might be true and other times it isn't. If you are worrying and think you should have added something and you want to send it in - you can do so.
