My son has a review pip firm to fill in he has ADHD and Aspergers he also has an immune deficiency meaning he does not produce his own antibodies he has a weekly infusion at home which I administer for him as he does not like putting needles in himself he is 30years old , he cannot travel on public transport as he is at risk of picking infections up .. due to his immune deficiency, I will fill his form in but I’m struggling how to word it can anyone help please, he doesn’t cook for himself I do it for him as his concentration is not good so he forgot a lot . Did you keep a copy of the original application form? if you did then use it as a template, especially if there has been no change in their condition, if there has been a change for the worse then explain what has got worse.

It is perfectly OK for you to type your answers and attach them as extra pages to the form, just make sure that each page has your son's full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

I would not split your answers across the form and attached pages, do one or the other for each activity (not question), splitting it risks the second part of your answer not being read.

Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
