Hello,

My 14 year old son receives middle rate care and low rate mobility for shortened calves causing mobility needs, ADHD awaiting assessment and moderate learning difficulties.

He has had a few recent episodes where he collapses and has a seizure. They have found an irregular heart beat so we are currently waiting on a paediatrician referral for this to be looked in to. So this is a new separate additional issue to what I originally put down on his claim.

My son cannot be left alone at all and is monitored a few times through the night, can’t shower alone so is assisted with this (waiting outside in an attempt to give him some independence) etc.

I have called DLA and informed them of the change. They have sent me a DLA1 form but the letter that is with this says to fill in the DLA434 form but they have given a DLA1 form.
Is this correct?

Is it worth me filling in this form all over again? Will they take his current care needs in to account?

I have tried to find the ‘change of circumstances’ form on your site but it’s not there.

Yes you need to complete the form they sent you, you need to explain how his condition has deteriorated and what additional care needs your son requires.

They will take into account what you tell them, so take your time and explain in clear language exactly what care needs your son requires. If you have new supporting documents so much the better, don't send originals as you will not get them back.

When you reported a change then you would have received a DLA 1 form to complete this was the form that you filled in when you originally applied for DLA, although it may have been called DLA 434.

Hi Rachel, sorry to hear about your son's episodes - my daughter has seizures so I know how it is. When you fill in the form again, my own experience is do not assume they will re-read or know the previous information you gave them, so you will need to tell it all again as well as emphasising the new care needs. So fill in every section not just the ones that have changed. Did you keep a copy of the previous form - I would suggest to start with that but then add the additional information on and explain all the extra needs at night, bathing, etc.
