New and old pip form I don't know which is mine was created by Xyz
Months ago I sent back a pip form saying nothing had changed in my medical conditions but I don't know whether it was an old or new pip form. I doubt that if I call dwp that they will know. I am thinking of asking for a blank pip form so that during the assessment I have something to refer to. Because since sending back the form my condition and medications have changed. But the form they send me might be new while the form they sent me months ago might be old. Or vice versa. What should I do, should I go by the old or the new form?

Hi Xyz, I would definitely phone PIP to confirm whether or not they got your initial form and what it was for. And then you would ask whether it was possible to add new changes to medications and conditions, because if not you may have to make a new claim, especially if they have affected your life a lot and the descriptors in the form would change through your claim.
