Hello,
Is it possible to claim both ESA and PIP without a GP’s input?
Ie you use your own medical records and sick notes as evidence? Or diagnosis letters and referral letters etc ?

Yes, it is quite possible to claim ESA and PIP without the specific input of a GP. You can even say on your forms that you don't wish the DWP to contact your GP. However, if you do this - it may be deemed suspicious. The DWP only contacts a small number of GPs, and it normally happens when it is deemed that the claimant has not provided enough information or evidence to support their claim.

As Bis explained, It totally is, many claimants these days don't have the sort of relationship with their GP as they once did in the past. That being said, If your GP practice has 'Online Patient Access' you can actually access the records yourself that they hold, in the form of 'Summary Patient Record' which tends to be fairly brief to 'Patient Records' which tend to be a little more detailed. In my case they span back 25+ years, but bear in mind that practices may vary if different online systems are available, mine use SystemOnline.
Going forward , It is a very useful source of corroborating information and supporting documentation that claimants are/have been in contact with their GP/nurse/health practitioner about their ongoing health issues etc. Also, it is possible to provide a history of medications that may have been prescribed as well as any referrals made to other specialists.
Although many people place significant importance on their GP's input, you are actually the best placed person to describe how your health issues affect you on a daily basis. It can be a daunting and overwhelming task and if you have anyone who knows you and your situation really, really well they can also support your application with their own written statement of support. Just remember, always bear in mind the PIP descriptors when answering questions, or providing supporting statements. Good luck AnxiouA.

Can I also ask, is this the same for ESA? They rarely contact the doctor and you’re the best person to describe how your health affects you?

I have not seen any states between WCA and PIP and contacting GP's. They do contact GP's but it is quite rare.
