I have an electronic version of the above ar 1 review form to complete and return online in a pdf format. it has been 'customised' by alternative formats team with my name etc . with the date of my claim JUNE 2018 no other codes as to version etc. it has 13 questions.
Briefly do we use the new pip guide or the older one? to complete the form.
As my son (who lives away) is completing he needs to know - with lack of space on this form - impossible not to split answers to questions - do we add an electronic (extra page) or do what was done for paper original app form - separate sheet (in another electronic file of evidence document in my case) cross reference in form "see evidence doc p ..... ? There is not enough room at the end with additional information to only use that last page in the document....
I have not yet completed an online electronic form yet.
I would use the new guide, it does not matter when your original claim was made.
If you cannot use PDF then I would contact DWP and ask for a paper version.
If you are using the electronic form then add extra pages as if it was a paper form, make sure that each extra page has your name and National Insurance number on them.
If you kept a copy of the original application form then use that as a template, don't forget to upload any supporting documents including one from your son if he does any caring for you.
