Hi, ive been sent a PIP award review form. The questions on the form are a bit different from your downlaod PIP guides(both the old and the new guides). the form is 25 pages long but the sheets to fill in take up only 12 pages. in the form ive been sent in section 3 there are no tick boxes (apart from mobility question),and for each question it states "tell us if something has changed and aproximately when", "tell us how you manage this activity now, including the use of any aids that you need", and "tell us about any changes to the help you need or the help you get from another person". have i been sent the right form? and im guessing its best NOT to write no change but to put the same info in again?
