ESA to UC - being forced to move to UC from ESA what evidence required

I have to make an online claim for UC as ESA is being ended in my area. Has anyone completed these forms and what evidence is asked in these forms with regards : medical, bank statements etc.

I've also read that they don't make you do another physical assessment, has this been everyone's experience.

Your claim for UC will be a standard one with the exception of how your existing ESA claim will be treated, as a Managed Transfer you will be entitled to a Transitional Payment if applicable.

The questions will be the same, see

www.gov.uk/universal-credit/how-to-claim

Your existing award or WRAG or Support Group will be carried over automatically to UC LCW or LCWRA respectively, however, the DWP do have the option of initiating a new reassessment at this time but this will not interfere with the transfer, from what we see on the forum this in not common.

The transfer should be automatic and they should not require bank statements
