Ok I have review Pip poem sent to me and don't know how to fill it in. I had lost few pages from the original form unfortunately so I can't just rewrite it. I have indication how many scores I scored previously or where I scored zero. What should I answer to questions where I scored zero previously? Also I had read that if you say something has changed they will stop your claim, is that truth?

So far I had managed to copy answers from previous form, the problem is that I don't have the answers to 4-5 remaining questions as that's how many pages are raughly missing.

Do you now meet the Descriptors for any of the Activities you previously failed to score for? If you believe that you do, then explain on the form your current limitations.

If you say that your limitations have changed then your claim will likely be reviewed with more intensity than if you had said everything is the same, but no, they will not just stop your claim.

What should I be replying in the boxes where I previously scored 0? Because I had no problems with for example toilet needs and what should I reply in a boxes that they scored me 0 even though I believe I had a problem.

Gordon my limitations are the same it's just that they have scored me zero for it and they do exist so I don't know what to put on the form.
