Hi Pat

Welcome to the forum.

You might want to have a look at the following FAQ which explains where everything is; www.benefitsandwork.co.uk/guides-for-claimants/faq/forum

I would ignore what the renewal form says, send in what ever supporting documents you have, make sure you only send photo copies as you will not get the originals back, add your name and National Insurance number on each supporting document you send.

Don't forget to keep a copy of your renewal form and send it in by recorded delivery, especially with the postal strike going on, a few days after posting check it has been delivered and signed for then contact DWP to see if they have received your form and that it has been placed on the system.
