DLA and suicide risk DMG chapter 61 references

I am filling in my son's DLA form and looking at the Decision Makers' Guide Chapter 61.

Suicide risk is mentioned in substantial danger paragraph 61175 which has a reference 1 R(A) 3/93
It also refers to paragraph 61154 with reference 1 R(A) 4/92.

1 Does anybody know what the references mean and where to find them? DLA government helpline tried, but don't know. It is not in abbrievations for DMG staff or for statutes.
2 Basically suicide watch is all the time but all nighters are not every week. Is there a way of putting this on the form so that it gets counted?

I've found R(A) 4/92, see

but I'm afraid I cannot find the text of R(A) 3/93 despite numerous searches

Thank you for the link. I twiddled with the numbers knowing as a typist that this could be an input error. I believe the correct reference is 1 R(A) 3/92 not 93 which I found using the link you sent and searching. This is a case of daytime watching over a schizophrenic suicide risk. It is interested to see the way the appeals tribunal thinks as this will help with my phrasing and reasoning on the form.
