Hi.

1. Is there a sample (example) of a supporting letter from main carer somewhere on this website?
If not, what points they should include in the letter? My condition hasn't changed apart social anxiety getting worse.

2. My carer will be filling in my form (Award Review). Who should be signing it and do I/we have to write my name and National Insurance nr and signature on every page like with the main form? Again, if carer is filling the form, whose signature should it be?

In case you are not aware your real name may be showing as your Forum ID, if this is the case then I recommend that you change it as soon as possible. See

1. I'm afraid there is not a template letter for this, everybody's requirements will be different so it is not practical to provide one, however, such a letter should include the following;

- Name and address of the person writing the statement.
- How they know the claimant?
- How often do they see the claimant?
- In what capacity?
- What prompting, assistance or supervision do they provide the claimant with, in the context of the PIP activities?
- What limitations have they observed the claimant have in context to the PIP activities? These may be the same as above or in addition to.
- They need to attest that the information is correct to the best of their ability.
- A signature

2. If they are completing it using your words then you should sign the form, if their words then they should sign it, in either case, you should explain why your carer is completing the form.

You should include your full name and NIN on any additional page that you provide, we also recommend numbering them something like page X of Y. We do not recommend splitting your answers across the form and additional pages it makes it too easy for information to be missed; again we recommend you stating on the form "see answer on page X of attached" and on the attached page, "answer to Question X". Do this last so you know where everything is.
