I was in exactly the same situation, i did not receive review letter or reminder and it was only when i rang PIP and asked why payment was less they explained, i was lucky they reinstated payment and posted a new form which i completed and returned with evidence by Special Delivery, i tracked it and phoned PIP a couple of days later to confirm they had received it and it was scanned on their system, yes it was, with regards to the Carer's allowance payment that was also stopped but PIP said they had informed them i was back in payment so it would be paid, well it wasn't, after my carer made 3 phone calls to them and some 10 weeks later she received the outstanding mone she was owed, they said they were busy!! just to let you know.
