Hi mine is 12 months in November,this whole process is so stressful and degrading,I was lucky enough to nurse for 40 years,until my back gave way and needed major back surgery,didn’t work and have had chronic pain now for over 9 years Now in addition to chronic pain I have early onset dementia,apparently there is a link between chronic pain and dementia because of all the medications I too have had 3 monthly text messages saying they have my form and are reviewing and I may still need an assessment
