Like I said in a post before that my medical records don't go into much detail
Like my last appointment I had I went in crying as I had been having acid come up all weekend and my throat was on fire I found it hard to even swallow ,
She put "thrush of mouth and oesophagus ongoing , HH playing up still , try fluconazole this time"

She didn't put down id been sick all weekend etc , because my surgery is "BAD" it has 1 star review on Google , ive even written in 2 complaints in the past ,
You can only make same day appointments and there considered "emergency" ones and u can only say one main problem ,

I feel if I don't put the medical records with my review , I will indefinitely have to have a f2f assessment, etc
I'm so confused wether to put them in or not ,

I did write a separate letter saying about the appointments and I'm unable to say everything and that I've had to lean on victim support (as I've had teenagers booting my door and harrassment with the street as one of the teenagers is related to someone on my street ,the police seen how freaked out I was (had an ex husband who was violent and would kick my door through (they were wooden doors then) so it made me feel frightened like I used to ,
So they contacted victim support to help me ,

Any help greatly appreciate , my grandad passed away 2 nights ago I've got my mum terminally ill in hospital) and my own health , plus my surgery is booked in for this Friday and my mind is everywhere

I'm sorry for the loss of your grandad and the distress you must be going through with your mum. It's a lot to cope with on top of your own difficulties.

I can't tell you whether or not you should put in your medical records. We always advise that people shouldn't include things that harm their case. Remember, these are doctor's notes and rarely include all the information from conversations, however much we might want them to.

I also want to say that including your notes does not guarantee that you won't be asked for a telephone assessment. (Face-to-face ones are still few and far between at the moment). You could put in a request asking for a paper-based decision explaining about your grandfather and your mother and your own surgery.
