PIP review form and extra pages was created by j112009
I will have to use alot of extra pages with the review form and have previously added them like this " please go toextra page 1 OF question 3 etc" and then said Question 3 continued on the extra sheet. I draw a line and tell them which part of the question I'm giving more info for.
Is this alright as I've already started the from this way.
Please advise. Thanks

Hi j112009,
Ther is no set format as to how to use extra pages. The important thing is to make it absolutely clear what question is being continued. Using page numbers, heading each question and perhaps also adding the descriptor titles, should help do this, as you have suggested. Make it clear that there is extra info and direct the decision maker to the relevant extra page.
The other alternative is to deal with question 1 solely on extra page 1, Question 2 is on all page 2.
This might clearer than eg having extra info for questions 1, 2, 3 etc all on the same page.
But...it doesn't matter as long as it is clear.
Really important is to include full name and NI number in all pages, and also perhaps the words PIP form continuation sheet page 1, page 2 etc.
