Hi Mandeep,

I have found it best to complete the review form as if you had never sent anything to DWP before.

So give full details of all your old conditions together with evidence (updated if possible).

Also give full details of your new conditions with relevant evidence.

It’s a real pain but at least DWP won’t be able to miss anything.

Hope this helps

Regards
Pete

Hi Mandeep

I know it’s a pain but I really strongly advise writing the information again in full.

You want to make things as easy as possible for the DWP.

Just in case you don’t know and don’t have a copy of your last application, you can request one from DWP then it’s just the sweat of copying across to the new form.

Pete

Hi Pete thank u for your reply however i need to send back my pip review form before 18/11 /2022 I dont think I have much time to get copies of my pip forms also this was my 2nd appeal for pip first time they had not given me anything then I applied again and had to appeal again however before xmass 2020 I had a phone call from dwp who said they were going to award me standard rate pip with 11 points just needed 1 more for enhanced rate but I was so tired and drained by then I just took the standard rate pip and they stopped my pip appeal and said that they were going to give me this award now they have sent me review forms I have only got the copies of the pip appeal I sent them can u let me know if I can send those again with the extra health conditions I have had after 2020 thank you

Hi K

You can use your previous forms as a template but not as an original form as they usually have a QR code printed on each page.

Use your original form as a template if your condition has deteriorated then add what has deteriorated and how it has affected you, remember that not all deteriorated health conditions causes higher awards, health conditions affect people differently, so you will need to explain how your condition(s) affect you.

If you need extra time to complete the review form, contact DWP and ask before the due date.
