hi there, i was wondering if you could advisee, my mother applied for pip , ( she used to get enhanced DWA), when she received the form , she really didn't give enough detail, and even mistakenly using "NO" and sent it ,
so very much underplaying her difficulties, i did explain this in my response and why she sent the form in a rush and really didnt think about what she was putting down or rather lack of information.
in my response, i put this all in a letter, with detailed information section by section, and a letter from her head nurse in charge of her rheumatoid arthritis.
so as expected , the mandatory reconsideration remained unchanged , claiming she put nothing on her form, and they pretty much used this in all the responses,
so naturally i will try an appeal that i have logged , so was wondering how i should out this across to them, as clearly they have not taken anything in my letter into account, and also failed to mention any reference from the health professional , that i enclosed in my response.

Hi rchan
I think probably the best approach is to get hold of the original form your mother sent in (if you didn’t keep a copy then you can request one from DWP) and then, following the same format recreate the form as you would have wished it to be completed originally.
That way it will address what the DWP are looking for in the style that they require.
Explain to your mother that she should tell the DWP about her worst days and that there are no good days just ones that are less bad.
Make sure you list every aid your mother uses including adapted cutlery, perching stools, tube squeezers, etc. Really anything that clarifies her level of need.
Describe how the illness affects your mother not just the symptoms. So pain is a symptom but may affect sleep or mental health or prevent activities etc.

thanks pete, i did exactly this as you said , wrote a long sectioned letter , and used subheadings for question 1,2 3 etc, even though i sent this at the mandatory reconsideration stage, and still they didn't really take it into account.
luckily i have a copy of the original form , that i used to formulate my response.
is it a good idea to amend this and send it as evidence on the online portal ? as we are at appeals stage. or ??? do i send a copy of exactly what i sent them, and make an additional observation document and send this in addition ?

When I first started out I used to do appeal as you have stated, spend time on doing the MR and then use the MR for the appeal if not successful at the MR stage, it worked ok.
Over time I have changed I how I set out my submission, now using the PA4, I set out what the assessor has awarded and then I set out what I believe should be awarded noting down the points and then explain as to why I believe this should be awarded, the why part is the important part especially if you can tie it in with any supporting documents. At the end I list number of points for the Daily living component and how many points for the mobility component.
I do the submission in a word doc then copy and paste it on the online appeal form.

The PA4 is the assessor's report - but this only applies if she had a telephone/video/face-to-face assessment. If the decision was a paper-based decision - then you need to request a PA3. Sometimes they will say one wasn't written - which I suspect they may say if your mother didn't put down very much. It's useful to get hold of a report because it gives more indication for their reasoning.
