I’ve applied for PIP (I did this online) and have my telephone assessment on 28th November. I have lost the information I submitted on my How your disability affects you’ form and as I had a stroke I suffer with memory issues so can’t remember what I said but I’m trying to prepare for it. Will the dwp be able to send me a copy of what I submitted? I’m frightened to ask.
I'm sorry this has happened. Don't be afraid to ring and ask. As you have only just sent it, I hope they will be happy to send you a copy. Explain that you cannot remember what you said because of your condition. Whatever they say, make sure that you keep a record - so if the form does not arrive in time for the assessment, you raised this as an issue.
