PIP review form was created by Tracy Fairchild
Hello to you all. I hope that you are all as well as possible. I wonder if anyone can please help.me
My husband is soon due for his PIP review, and I was wondering if someone may happen to know where I can find a copy of the up to date form, so that I will know what questions will be asked, and will be able to start the form as well. I would be truly grateful for any help that may be given. Take good care everyone. Kind regards Tramelia

Hi Tracy
If you look at your latest guides it will give you an indication as to what the review form looks like, it will also give you hints on how to answer the form.
I am not aware of where you can find an example copy of the new form, you may want to try and google it, forum members may be able to point you in the right direction.
I always to claimants, treat the review form as though you were making a new claim, if you kept a copy of your original form then use that as a template, if your condition has deteriorated then adjust the form accordingly.
