Hope someone can answer this simple query. As I can mobilise safely no more than 20m is it OK to leave the boxes 50m, 100m, 200m It varies unticked and explain why in the extra information space. Thank you

Just remember, the ESA Mobility test includes the use of a wheelchair!

You do not need to use a wheelchair or to even own one for it to be considered so you must also explain why you cannot self-propel such a chair for more than 20m and certainly no more than 50m, assuming that this is the case.

I have been given an attendant -propelled chair for safe mobilisation following an OT assessment. I cannot self propel due to my upper limb disability. Will the OT report be useful supporting evidence for this activity ?

Yes the report should be useful especially if it explains why you cannot propel a wheel chair i.e no strength in your hand/arm, or no grip in your hands.
