Hi,
I had a Paper Based Assessment carried out by Independent Assessment Services.
I Would like to request a copy of the assessors report, is it still called a PA4 for a paper Based Assessment. ?
Do I ask the DWP or IAS for the assessors report ?
Should I wait for my Decision letter from the DWP before asking for a copy of the assessors report ?

Hi Kris2fer
If you had a paper-based assessment, it is a PA5. However, be aware that many people who have paper-based decisions are being told that no report was written - so there may not be one available. It is the DWP who will have a copy if one exists.
