Wording Multiple Conditions Describing Problems with an Activity on PIP2 Form

Hello

I am helping my Brother with compiling a claim for PIP (it is taking a very long time because he is anxious and paranoid because of the stress caused by his first failed claim that took 2 years) he has multiple conditions, kyphoscoliosis (combination of kyphosis and scoliosis) severe spinal osteoperosis, multiple spinal compression fractures, osteoarthritus in both hips and recently diagnosed with joint hypermobility.

My question is how best is it to word multiple conditions when describing the limitations doing the PIP activities on the PIP2 form. Like for example what we have done on our draft activities (we have done many drafts) is this :-

"I am frail, weak, unsteady and have chronic pain and discomfort because of severe osteoporosis in my spine, hypermobility in my joints, osteoarthritis in my hips and spinal curvature caused by kyphoscoliosis. I need to use the aid of a....."

Is there a way to word multiple condidtion without having to repaet them constantly thoughout the same activity to describe the condition that causes the restriction.

Your brother and yourself are the people who best know his condition and how it impacts on his life. You want to make it clear the difficulties your brother has and how much he needs support and at the same time make it as easy as possible for the person reading it to understand (It can feel like an impossible challenge!) This is just me - but I always include an extra page with a table listing all the medical conditions in one column, the symptoms in the second column, the amount of pain in the third, medication in the fourth column and PIP questions they are relevant to. You don't have to do this, but it helps to give a clear picture of what is going on. Remember one of the people reading this is a non-medical person.

Then when it comes to the question, it becomes labour-intensive if you have to keep repeating every single condition for each thing. If you do the extra page - you can always list one or two and say - "see chart xxx." However, you can also do as you have done and tell them all, but only use two or three in your answer. All of the conditions won't cause the same strength of symptoms all at the same time, so you can choose wisely to the ones that feel most relevant.

I say all this, but at the end of the day, you have to do what feels right for you both. As you know, the whole PIP system is fraught with difficulties, and it's rare that anyone sends in a form and is entirely satisfied with what they submitted. Two people could read the finished form and have very different views of it.

Hi Waxwing,
Think about the cumulative effect of the health conditions. So for example, does hypermobility cause fatigue? Does that cause the scoliosis in the spine to be more pronounced? Perhaps you take medicine for one condition which affects another? If the hands are affected does that mean you you have poor grip and can't use a stick?
Is there a mental health issue that arises due to the physical health or gets aggravated by it?
