My wife submitted her first ever PIP claim for RA and has just received a text informing her that a telephone assessment would take place.
Can anyone advise any help for her as she gets quite nervous.

If you look at the Guide to PIP claims and reviews, a whole section covers telephone assessments, how to prepare for them and the questions she may be asked.

Hi Maddymoo. Just to emphasize two key points as well as all that BIS has pointed you to in the Guide.
Please please phone the assessment provider ASAP - when you get through [first thing in the morning usually the shortest wait, but always have a pastime for while hanging on] - you can put your wife on just to confirm her identity and that she wants you to speak on her behalf. Then inform them that she wishes to have the assessment recorded by them. This may get the reaction that she will have to wait longer. Don't be put off, please. No one should risk trusting to an assessment that is not recorded. Due to the amount of 'problems'(!) they have with recordings, also covertly make your own, totally legit. for own use.
Secondly, you can be with her throughout, advocate for her if she needs a break. She could say at the beginning that she is anxious and wishes you to help her answer. They may not like it but the two of you should stay in control and not let them bully you as we often read on here can happen.
If the anxiety would be too much, you can ask the GP or mental health professional to write saying a phone-call would cause Overwhelming Psychological Distress [OPD in DWP-speak] and send this in with a request for a paper based assessment instead. But then you don't get to speak.
