Data Subject Access Request was created by Doofa
Hi all.. wondered if anyone could help in relation to pip. Mid 21 pip ran out and review paperwork provided and returned. Feb 23 tel assessment awaiting hearing back currently. I asked for a copy of HP assessment and it clearly contains things said missing and not said contained. Which can prove. I overlooked recording request. Point is based on that info I will be doing a MR when receive award if any. I called IAS and asked for their recording, they said not recorded because I had not requested. Which if true means the assessor must have just taken notes.
I would like to request a DSAR for those notes formally.
Does anyone know who or where you submit a request under DPA. Thanks.

Hi Doofa
You can contact the DWP for a Subject Access Request. All information about your assessment should have been forwarded to the DWP. I expect them to say there is only the report and no additional notes! The fact that your report had things missing from what you said or has incorrect information is quite common and does not necessarily mean there are other notes.
We know from experience that assessors if they do a couple of assessments close together and then fill in the information later, it is not always right!

Thank you BIS,
If i do a MR could I provide a verbatim response to the questions asked that will not have been considered because the HP failed to document important information that would clearly change the outcome. Eg I need help washing my hair and told her I do and why but has herself scored me nil on the sheet along with many other missed information.
Thank you

You can present whatever evidence you think is relevant your MR request, however, I would be cautious of presenting slabs of text to the Decision Maker without, at the least providing a summary of what you are including.

Thank you Gordon.
If info I provided not handed over in the report received, should I not refer to this when judging from the report I've noticed where points should have been awarded the HP has not in her report which is an identical pip form. It appears to direct the dwp decision maker where they should award points..
So if they do not award points where points should be should I not provide details of what I provided to the HP in regard to each section.
I'm a little confused.

I think you have misunderstood my last post, it is perfectly OK to do what you are suggesting, all I am saying is that if you are going to provide large amounts of text for each activity then you should consider including a short summary at the beginning, this will help lead the Decision Maker into reading everything you have to say.
