Requesting a copy of the assessment was created by RHAN
Hi everyone
Could anyone tell me how to request a copy of this sort of assessment please?
Bit of a difficult one, and alot of messing about going on with my claim pack.
I’ll jump straight to the point.
I was called out of the blue to do a phone call assessment when I was already granted a paper based one atleast twice previous to it because of my Chronic conditions and struggles with pre booked appointments. Ive failed to even answer 3 phone call assessments because of my conditions so was granted paperbased twice after this.
This random call basically lasted no more than 1:30m as I was struggling and failing it due to still recovering from a severe episode of my condition lasting days, and the lady HP assessor realised this so she said she will leave me be and will go back to paper based.
There has been no decision made yet, its been 3 months since that call.
There’s a bit more that’s happened within this 3 months but I just need to get a copy of what happened that day.
So could anyone tell me how request a copy of it and a copy of what do I to ask for as its a bit of a phone call assessment & into a paperbased assessment I presume?
Is it also normal for DWP to assign my claim(going on a year now) to a new HP assessor after that incident?

Thanks for getting back to me.
Ive read your reply thank you for the information I needed.
It was a bit of both, started as a surprise phone call but quickly reverted back to paper-based as I just couldn't do it. So I presume I need to requestI both PA3 & PA4 reports in one.
Could you also tell me if its a normal thing for DWP to assign a PIP HP assessor
(who attempted a surprise phone call assessment but had to revert back to paper based as i failed it which was 3 months ago) only to switch it to a new HP assessor and for him to conduct another pre booked phone call assessment ?

On the forum we have heard of a number of members who have received a surprised phone call, you ask is it normal? My answer from what I have read on the forum is yes, as more claimants are now receiving surprised phone calls, which does not go down well with their mental health.
I have not heard from members who have received both PA4 and a PA3, I would assume that if the assessor could not complete a telephone assessment and had to revert to a paper assessment then it would be a PA3, but nothing would surprise me nowadays.
Let us know which report you received.

They would assign you to the first available assessor, so I would not read anything into it.
