Hi David,, I was mistaken, the form I found was a medical services appointmenton behalf of dwp that got cancelled. 2013, I believe my dr would of wrote to them

Hi Lisamaria,
As you have been diagnosed with bipolar disorder and agoraphobia you could make a very good case as to why you are completely unable to travel to a face to face and also being too mentally debilitated to participate in it.
You need however supporting evidence. I am not aware that you have a Support Worker. A lot of Mental Health Services use Lived Experience volunteers and maybe if you phoned up local MH services they could allocate you an assistant to write some supporting evidence about the effects of your condition.
Otherwise you could ask for help from a CPN through the GP surgery ( community psychiatric nurse ).
If a CPN is not available the surgery should refer you to a Mental Health Practitioner ( see link below ).

Not related, but just had our council tax updated as we moved over to uc,, we now owe over 500 quid,, we don't get the same reduction is this normal?,,

Hi Lisamaria,
I cannot comment as every Council ( Local Authority ) is allowed to make the terms of its Council Tax Reduction scheme.
So you will need to ask them the reason for that £500 difference.
