PIP assessment report abbreviations

PIP assessment report abbreviations was created by oak1561
I got the assessors report for my telephone PIP assessment but there are a large amount of abbreviations that I don't understand. I want to ask for mandatory reconsideration but it's hard to understand some of the report due to the amount of abbreviations included.

I checked this list : www.a1benefitsupport.com/commonly-used-abbreviations-by-dwp

But I couldn't find any of the following:
IO
FH
HOC
CMHT
MH
MSE
SOH
OPD


Example sentences:
The evidence in the FH showing reported to be...
MSE unremarkable and SOH showing working...
HOC showing no CMHT or CPN input for MH, IO showing no low mood...
No anxiety or OPD evident

Can anybody help me out? Thanks

I will try and answer some of the missing abbreviations, any that I don't know, forum members will be able to answer:

IO ~ Informal Observations
FH ~ Functional History
HOC ~ History of Conditions
CMHT ~ not sure on this one CM could mean ` Current Medication but not sure HT could mean History Treatment but I am only guessing
MH ~ Mental Health
MSE ~ either Mental State Examination or MusculoSkeletal Examination or Medical Services Examination depending on context
SOH ~ Social & Occupational History
OPD ~ Overwhelming Psychological Distress

CMHT is "Community Mental Health Team"
