I absolutely would ask foe them to change to a phone assessment, saying that you cannot travel to the assessment centre as you have no one to take you and because of the pain you are in.
Undoubtedly, if you go to the assessment centre, they will ask you how you got there. If you say that you arrived by train they will know you left your home, travelled to a train station, caught a train, left the train station, travelled to the assessment centre and walked into the assessment room. There is a huge risk that the assessor will not understand the issues you have with your back (unless someone is pushing you in a wheelchair). It is difficult for assessors to judge a claimants pain, but they can make assumptions about what they see regarding someone walking.
If, for some reason, they turn down your request - you are entitled to make a second request. Most people have found the assessment services are willing to change the type of assessment.
