SR1 Form was created by AndyWalker13
Hi

I hope someone can give me some advice. Unfortunately following many tests/scans, my wife has been diagnosed with stage 3 lung cancer. She is quite poorly and we only saw the consultant oncologist yesterday along with one of the MacMillan nurses. She cannot have surgery or radiation and will commence chemotherapy in the new year. Our MacMillan nurse told us at the end of the meeting my wife would be eligible for PIP and she would sort out an SR1 submission and Blue badge for her. I came off the phone about half an hour ago completely confused. We had just taken a call out of the blue from Citizens Advice telling us to apply to the DWP for a PIP form by phoning them. He told us he had an SR1 form at the office but also need us to get a standard PIP form. I was under the impression the only form needed was an SR1 form which was filled in by doctors and MacMillan nurses and would not require both forms. Please can you advise because the chap from Citizens Advice sounded a bit unsure and we were also under the impression they would be filling in and submitting forms for both PIP and a Blue Badge. Many thanks Andy

Hi AndyWalker13

They are both correct, when a SR1 form is completed, that entitles you to the enhanced Daily Living component of PIP, if you also want to apply for the PIP mobility component then you have to complete the relevant sections on the PIP form.

If you are claiming PIP under the special rules, you will be asked questions about your mobility during the phone call you make to start the claim. This should speed up the decision on the mobility component.

Hi Gary
Thanks for replying. Just so I am clear, I am to contact DWP to explain my wife's cancer team is applying for PIP under 'special rules' separately but I am asking them to issue the standard PIP form so we/Citizens Advice can complete the section on mobility? And if MacMillan are using Citizens Advice to complete these forms, I assume they will be helping us to complete the mobility element and submitting both forms themselves on my wife's behalf. All I know is the SR1 has to be completed by medical staff so why have we had a call from Citizens advice as they cannot fill that form in. I just assumed my wife's nurses were doing that side of the paperwork.

Hi AndyWalker13

Yes you are correct, some areas now have access to complete PIP forms online. Citizen Advice will be able to give you further information.
