Dear BnW,

I got a request from ESA asking me to send them last 4 months Bank Statements,ID evidence and my recent job search.
to send all by 03.04.25

I actually have LCWRA.
It's unlikely i'll be able to work in near future.

My health has been significantly worse since begging of Feb.

I want to know for a fact if i need to fill to fill in this form?
I don'k know if i'm actually suppose to fill in the form?

Can you please clarify?

I think you need some personalized advice regarding this. You could phone Citizens Advice Help to Claim.
