Hi,
For various reasons I'll find it practically impossible to go to a jobcentre to deal with my ESA>UC migration (I can't verify my ID and I'm now awaiting 'a call' from the DWP). CAB have advised me to request a Reasonable Adjustment in my Journal but I'm unsure of how 'formal' to make it. I'm considering the following:
"REASONABLE ADJUSTMENT REQUEST DUE TO COMPLEX NEEDS
Please be aware that I am migrating from irESA (Support Group) to UC and have complex needs due to both mental and physical disability.
Due to both physical mobility issues, and sensory processing issues associated with Autistic Spectrum Disorder, I find travelling to and being in unfamiliar places extremely stressful. Such activity causes me such emotional distress and physical discomfort that my conditions increase in severity, often for many weeks.
Autistic hypersensitivity and social phobia around face to face interaction with a stranger may cause sensory overload, leading to overwhelming stress and confusion. I cannot reliably retain or relay information face to face to a stranger, and any responses given by me under such circumstances would be unreliable and compromise a fair assessment of my circumstances. I can only interact effectively on the telephone and in writing, because I cannot see the other person.
Unfortunately at the moment I have no support available to enable me to engage even superficially on a face-to-face basis with a stranger. I therefore request that, as a Reasonable Adjustment under the Equality Act 2010, my claimant commitment is discussed and agreed by telephone."
I'm just trying to be as thorough as possible in explaining my situation. Is that way too much information or is it OK?

As a Support Group claimant it is your right to request an alternative to commuting to the Jobcentre.

Is the above statement too detailed and formal, or is it OK to just post it in my journal like that? I want to assert my rights but I also don't want to start my 'relationship' with my Work Coach on anything that could be seen as an antagonistic note.

I would go ahead with your script.
Never be timid with a Work Coach or before you know it you will be recruited to one of the Jobcentre programmes in the link--
