Hi all,
Last week Tuesday was all a bit of a rush to get the form submitted particularly as I did not know there is a 5000 character limit to all parts of section 3 and other sections. So I had to do some last minute editing for about 3 sections. Perhaps that might be a good addition for the next PiP guide revision which was fantastic by the way.
However I submitted it one day before the deadline, but I do not remember Section 4 displaying at all after submitting.
Also on uploading the from & evidence I was not asked if I wanted a copy of my form. I think the screen went straight to the uploading evidence section which I completed too. When that was finished it asked me if I wanted to log out I recall.
“Once you have uploaded any supporting evidence you can upload your form and you will be sent a
confirmation email. On the screen that tells you your information has been sent you will be given a
link to download a completed copy of your form in .pdf format.”
I just called the DWP PiP line and they are sending me a paper copy of my form in the next 10 days and have made a note about times that I am not available to take a call, but they cannot guarantee that this information will get to Health Assessment Advisory Service (HAAS).
The DWP lady said that they had just made some changes to the form but she had never heard of section 4.
I was also just now able to get through to Ingeus who made a note of my preferred contact number and times as well as assessment type.
Has anyone else had these issues with section 4 and being asked if you want a copy in .pdf format?
