Hello,
I wonder whether someone would be able to advise me on filling in the 'Request for Personal Information (Subject Access Request)' form?
I'd like to ask for a copy of my recent EMP medical report for DLA.
In Part B, they ask you to tick the box of which benefit (DLA, etc) you're referring to, but at the top of the list, it states 'Computer records' and 'clerical Records'.
Can anyone tell me what difference it makes to what is sent to me and which I should tick?
In Part C, they ask for names & addresses (and if possible 'phone numbers and names of staff you've dealt with) of DWP offices which may have my records.
Surely on seeing my name, address, Nat Ins No. and the benefit I'm referring to, they would already know which of their offices would have my records. I'm not certain if it's Blackpool or Belfast (I've had letters from both).
Who exactly keeps copies of EMP medical reports?
Then, at the end of the form it states to send or take it to your nearest DWP office. I don't know where my local office is, and would have thought the place to send this form would be to where my DLA claim/medical was handled. Blackpool or Belfast? I live nowhere near either!
Any help would be very much appreciated.

Hello Bunty,
I recently asked the dwp for a medical report. I rang the Blackpool office on 08457123456, answered some security questions, asked for report to be sent and within a couple of weeks I received it. No problem. If only everything was that easy with them.

Bunty the DWP are the data controllers for your info..I suggest just sending it by post to Blackpool. You don't have to get too involved in the details, again why not just tick both the clerical records and computer ones.. its interesting what they throw up.. Like you say, they can soon trace your records so don't worry too much about the extra technicalities, your request is still valid, just make sure you sign it..

Hi, I've just sent for a copy of all my DLA records, just wrote them a letter, asking for what I needed, with dates from and to. That was on the 26/04/10, and I have today received a reply, saying that they will be sending the information ASAP and withinn the 40 days of receiving my letter. I just sent my request to Blackpool.

I don't know if it helps you but the Data Protection Act says a Subject Access Request does not have to be on any particular form. (However a form may get processed more quickly if the staff aren't familiar with how to process a request in a letter.)I wonder whether someone would be able to advise me on filling in the 'Request for Personal Information (Subject Access Request)' form?
