DLA claim form

DLA claim form was created by emmandelo
Good morning. I have just about completed the DLA claim form and am almost ready to send it off. My wife is about the only person that gives me halp and care. I have referred to her in practically every answer I've given on the form. Does it still need a statement from her or would that just be a bit of overkill.

Hi emmandelo,
It's quite hard to manage overkill when it comes to DLA. I would be inclined to include a short statement from your wife cnfirming what help she gives you.

Give as much information as you can, just make sure your wife has put all her details down on your DLA form as they can contact her good luck, hope you get it.

okay, thanks. She has filled it in, just repeating everything I've already said really. Am posting it off recorded delivery tomorrow. I've put on it that if they need to contact me can they do it by letter as I'm no good on the phone, get very confused and flustered. i think they already have my number though. Having asked for contact by letter are they likely to still ring and ask for my wife.

you signed somewhere saying they can contact people for more information on you, so in theory (i think) they could contact your wife direct if they wanted more information on what she has put in her box as your carer ?
