How condition affects me

Hi,
I am currently on appeal against ESA decision, awaiting tribunal. The DWP have never contacted my doctor, my problem is that if they did decide to do so, the lack of information my doctor has with regards to how my condition affects me day to day could go against me.
I have had to spinal operation (lumbar region) suffer sciatica, low back pain, hip/buttock pain and calf cramps. I have never been asked by any doctor how this affects me.
Should I just write a list of how it affects me and ask my doctor to put this on my records? Everytime I see a doctor it's a new one, can't see the same doctor every time so not had the chance to build up any sort of doctor/patient relationship.
Would making a list be acceptable?

Hi,

There would seem nothing to be lost by doing so. However do be aware that the DWP may or may not contact your GP if they feel they do have enough information to make a decision on your claim.
