Just finished my ESA50 - Help!

Just finished my ESA50 - Help! was created by Clarity
I have to return my form by 6th December and began filling the downloaded form in as soon as I received the phone call.
I needn't go into the stress and dark thoughts that have decended on me, and my CPN has but needless to say it is done.
The sadly my CPN, who assisted me, used a large part of my words from the form in my care plan which she sent to my GP, though she did state it was "congruent with" what she had seen. She also said I had taken too much medication over the stress at the end of the form.
I will be seeing my GP tomorrow, what do I ask him?
Will any of this count against me?
Finally, to whom should my carer hand deliver the copy of my ESA50 at the local Job Centre Plus? Should the envelope say, for example, Decision Maker?

Picking up on your last point first, the ESA50 needs to be returned to ATOS Healthcare not your local JC+ office, there should have been a pre-paid envelope with the correct address on it, that came with the ESA50.
You say you are seeing your GP, but not why. Are you doing this to update him about your ESA claim? Will you ask for his assistance?
You could show him a copy of your ESA50 and ask for his comments.

Just a point about returning your form - if you have too much paperwork to fit into the provided envelope, you can always tape this onto the front of your own envelope - thus allowing you to take advantage of the 'prepaid' component

This is the third time I have sent off an ESA50 & extra information to Atos with the prepaid envelope sellotaped to the front of my own envelope & the first time postage was charged. The guy behind the PO counter said the free postage didn't apply if the prepaid envelope was attached to another envelope!! Didn't have this problem when I did the same with my DLA claim form earlier this year.
