Thanks, in the document you've recommended I look at . I've just read the answers to my questions today, from Harringtons own report. It states that "decision-makers (should) receive training so that they can give appropriate weight to additional evidence." Which begs the question , why was that not the case from the beginning! And, if dms are at different stages of that training process, where is the consistency within the system...?
