Most Specialist, Consultants, Therepists, etc. will usually write to your GP after each appointment so you should only need to get your medical records from your local GP practice, although you can make a request to your hospital as well.
See the following FAQs

Charging for medical records

I wanted to ask about supporting evidence, i received a letter to say i had been placed in the WRAG group without having a medical when i had hoped that i would of been placed in the support group, i have sent my appeal form in this week stating that i am unable to walk 20 metres without having to stop and rest because of the amount of pain im in.I have been told off my surgeon that i need a knee replacement but im to young to have one yet, i have asked my gp and physiotherapist for letters of support, and even though they agree with me that i can no longer walk without severe pain, they both said that they do not do letters any-more for appeals, so i was wondering what i should do for further evidence for my appeal? do i ask for copies of my medical records from them both as i don't know if that will help at my appeal. thanks for any advice.
Hi SL,
I note that you specify that you are "unable to walk 20 metres".
Please remember that the mobilising descriptor also includes the use of a manual wheelchair unaided, even if you don't usually use one.
Please see the WRAG (LCW) and Support Group (LCWRA) descriptors here :

Also as Gordon says, you can request that your Physiotherapist, Consultant, or other treating HCP copies you in on any post clinic letters to your G.P..
This should be done free of charge, if you have any problems with this request, and you attend at a hospital, contact PALS who should be able to sort it out for you.
cheers
