Statement of fact was created by Metallichick
Just a quick question? I wrote my doctor a statement of fact out lining who my illness actually affects my day to day living, so that kind of information is in my records. As my DLA will run out at the beginning or sept 2011 is it worth me writing an updated version for my medical records as i'm sure i'll be sent a re newal form closer the time??

I never thought about doing what you have done, but to me it sounds such a good idea, as most GP's are totally unaware of ones problems and this is a sure way of keeping them in the loop and allowing them to see what meds/procedures etc could be implemented to help one.

Nothing on this board constitutes legal advice - always consult a professional about specific problems

There's no reason why not, but be cautious with your phraseology as you don;t want your GP to simply replay your words, and worse still, credit them to you, on any report he might produce for the DWP, as this will not be in your interest.Just a quick question?
