Odd Child DLA form received. Help please

Requested a DLA claim form for my son from Blackpool recently, subsequently received. However, due to my own difficulties always use PDF versions, and print and send this with the paper claim form.

Have a problem with this however. The claim form they have sent me is 'DLA1 Child P November 2011'. The previous PDF version I have is 'DLA1A Child February 2011', and the new version is 'DLA1A Child January 2012'.

Please can someone explain what this form means, as I have never seen one like this before.

I'd imagine it's a revised form of a Child DLA claim form. That was first issued in January this year.

However I'm sure a moderator will no better.

Thank you. My problem is use voice activated as problems writing, and wasn't even aware there may be any different forms, so this menas just more delays.
