I wonder if anyone can help with the link Gordon put up for the pdf file. When I try to download it it tells me that it's the printable version, rather than the interactive one. Does anyone have the link for the interactive version of this form as I can't hold a pen anymore and need to type instead?

Can I remind everybody that this link is for the ESA1 claim form, not the ESA3, whilst sections of it are supposed to be the same as the ESA3, you need to be careful that the DWP do not assume you are attempting to start a new claim by completing this version, as this is likely to cause more problems than it solves.
