The form, an ESA113 is downloadable, just Google it, however, legally, only the DWP can submit the form to your GP and/or Consultant.

It should be noted by members claiming benefit requiring medical reports that the DWP are under no obligation to seek medical reports, although if they are needed then the Decision Maker will often request Atos Medical Services to carry out an examination as DM's are not medically qualified.

However, the onus to supply additional medical evidence lies with the claimant and not with the DWP.
