My present award of DLA ends on 21st June this year.

I dutifully sent back the claim pack, ( looked exactly the same as a new claim pack, which I'm sure it was). I used your guides and filled the form in as fully as possible and enclosed a support letter from My GP, with the form.
This was received at Blackpool on 12th March.
On 10th April I received a letter saying that they'd requested a report from my GP. (Subject of my last thread).

I then received another letter on 25th April saying that they'd requested a report from my consultant.

I phoned Blackpool on 25th April asking them to send me a copy of the GP Report. The guy I spoke to said it would take between 7-14 days as they'd have to photocopy it.

So I waited patiently but no report came and today I phoned Blackpool for an update.

I was told that it hadn't been sent, as they don't send them until a decision is made.

While I was on the phone I asked if the consultants report ( I have 3 consultants, all at different hospitals) had been received by them.

The woman I spoke to said it hadn't been received ( Isn't there a time limit) and told me which consultant they'd sent the request to.

I then phoned the consultants secretary, luckily, I had her phone no.

She is unable to find the form and my consultant is in the USA until 28th May.

To cut a very long story short, I had been seeing this consultant registrar since 2003, appointments were usually every six months, but kept being cancelled due to her illness and then eventually she retired, the last time I saw her was 24th November 2010.

Then I sort of fell off the end of the list, and was referred back to the main consultant by my GP as my condition has worsened and have an appointment with a new (to me) registrar on the 6th June.

The consultants secretary, suggested that I ask the DWP to fax through the details to her and she would ask the new registrar to complete the report, based on my old notes.
I'm now worried sick, two of the conditions I have are made worse by stress (Severe IBS and Angina), and I've spent all morning either on the phone, on the toilet, or using my gtn spray.

I'm at my wits end, as if I lose my HR mobility I'm going to be stuffed.

Has anyone any advice.

I guess the question you need to ask yourself is whether having this report available to the DWP is to your advantage or not.

If you do, then I would go back to the DWP as suggested by the secretary and try and get the form re-sent so it can be filled in.

If you don't feel that a report from this consultant will add to your claim, then leave it be. If the consultant does not answer the request, the DWP will eventually make a Decision without this information.

With regard to your GPs report, the DWP do not usually make any of the information used in making a Decision available to the claimant until after the DEcision is made.
