Complaining to my MP About DWP

Complaining to my MP About DWP was created by Dalglish60
I won my ESA Appeal without having to attend the appeal hearing as my appeal was revised in my favour before reaching the appeal date.
I initially wrote to my local DWP office 11 weeks ago to request a revised statement of reasons, revised decision makers formal scoresheet,formal award letter and revised medical reports. I have received nothing and after speaking to them direct on the phone 2 weeks ago they claimed they had not received any written requests from me sent by special next day delivery post. I have all post office receipts confirming delivery through track and trace.
The only thing I have received after speaking to them 2 weeks ago is a formal new award notice letter with current benefit rates and how it's worked out etc.
I have had enough of them telling me they have not received respective written requests from me etc.
I know who my MP is. When writing to him to complain about the DWP and this exhaustive delay do I simply outline the chronology of delay after delay.Alongside the fact I have a right to possession of the requested documents so I can refer to them when my ESA is up for review.
Or is their a standard template letter on this forum that is accessible to members who wish to write to theie MP about the DWP ??

I am afraid that there aren't any templates for this type of letter on the site, what you have suggested seems fine to me.

The LT54 document contains the decision makers reasoning for the revised decision and probably the best way to obtain it is to make a subject access request for all of the documentation relating to the decision (in case there's any other relevant documents).
There won't be a revised medical report. They will simply note on their system that the medical report was overturned on appeal (same as labeling it useless!).
