am going through blue badge application form

there is question

how do the condition described above affect your ability to walk

can anybody help me to phrase the effective answer to this question. i suffer heart disease angina and arthritis i just canot get my head round to answer this question

what happens when i walk ?

i experience pain and discomfort in my legs after walking short distance. also feel breathless and sometimes feel very tired and week afterwards.
